Covid – 19 Wellness Questions

 

  1. Has your child been sick or displayed any symptoms of an illness (vomiting, diarrhea, runny nose, cough, fever, rash, etc.)?
  2. Has your child taken any medication, including any fever reducing medications (Tylenol, Aspirin, Motrin, Advil, etc.) in the past 24 hours?
  3. Have you or your child been exposed to someone with Severe Acute Respiratory Syndrome, Coronavirus 2 (SARS-CoV-2) / COVID-19?*
  4. Have you traveled out of state in the last 14 days?
  5. Have you been in close contact with someone who has traveled out of state in the last 14 days?
  6. For staffing purposes – using a 20-minute time frame, what time do you expect to pick up your child from the facility today?

 

 

If your answer to any of the first three questions is “yes,” your child will not be allowed to be dropped off for childcare. So, please keep them home and do not even bring them to the center. These questions wellness questions are part of the state mandated covid -19 action plan. Any person who answers these questions in a dishonest/untruthful manner will be reported to the appropriate state authorities and:

  • May be prosecuted.
  • Will be held personally liable for any outbreak of the Severe Acute Respiratory Syndrome, Coronavirus 2 (SARS-CoV-2) / COVID-19.

 

If your child is ill or sent home with a fever, cough or sore throat, he/she may return following 2 consecutive negative Covid-19 tests taken 24 hours apart – or – after 10 days of being fever free and 72 hours of being symptom free.**

 

 

*The name of the virus that causes the Covid-19 illness is called Severe Acute Respiratory Syndrome, Coronavirus 2 (SARS-CoV-2). Covid – 19 is the active illness or manifestation of symptoms in a person. Please remember that over 50% of the people who carry the Severe Acute Respiratory Syndrome, Coronavirus 2 (SARS-CoV-2) show no symptoms. They are asymptomatic but they are also carriers of the virus, can pass it on and can cause a deadly outbreak of the disease with catastrophic consequences.

 

** Symptoms of the Severe Acute Respiratory Syndrome, Coronavirus 2 (SARS-CoV-2) / COVID-19 are any of the following:

  • Fever
  • Sore throat
  • Cough
  • Shortness of breath
  • Muscle aches
  • Loss of smell or taste
